I'd say that the doctor or nurse who physically examines or treats the patient knows far more than a nurse in another state who approves the charges in EVERY case although, having a nurse reviewing the charges or a Doc or a PA under Obamacare is a silly pipe dream since it will more likely be a GS-5 clerk who approves or denies the charges, followed by a GS-9 reviewer with no medical training who reviews the appeal and only after a similar series of bureaucratic malarkey will an appeal finally get to a Licensed Medical authority. Note that I did not say qualified medical authority but Licensed as the final arbiter will be that panel with some representative Docs or Nurses who haven't touched a patient in a decade or more and beaucoup admin types. Each case will likely have about a 15 minute review by one of the board reading the initial request and the appeal and then a vote. One day it will be found to be cheaper by Government to spend a Billion or so in loose change to some software company to develop a program with medical algorithms built into it which will render a decision based on the garbage input into it by someone on the 15 member panel. Of course before it's fielded it will undergo several revisions and changes and eventually closely resemble the companies trademarked Commercial Off the Shelf software which the company used to supply to medical insurance fly by nights like Humana and Kaiser et al for $150 per individual license for each computer using the program but the costs will have risen far beyond the originally bid Billion or so because of the intransigence of the US Government to accept the COTS program without some modification (minimal) Want proof of that part of the scam…1) look up the DOD DIHMRS program which Congress mandated would be fielded by 1998. It is STILL in the pipeline and only very small portions of it have been fielded with those only appearing in 2006 or so. 2) Look within the Obamacare Bill and see the extent implied of computerization of the Obamacare Law.

perhaps the far away in another state nurse that approves the charge for medicare knows more than the doctor who deals with the patient... Or Not... Do you even know how the Medicare billing system works???
